This test includes total testosterone performed by liquid chromatography/mass spectrometry, the most accurate testosterone test and one without an upper limit. It does not include free testosterone.

This test includes total testosterone performed by liquid chromatography/mass spectrometry, the most accurate testosterone test and one without an upper limit.

This panel does not include free testosterone.  You can order total and free (LC/MS) using this test instead:

Testosterone, Total, LC/MS and Free (Equilibrium Ultrafiltration)

Method Used for Total Testosterone:

–   Liquid chromatography tandem mass spectrometry (LC/MS/MS)- No upper or lower limit

–   Analytical sensitivity: 1.0 ng/dL

–   Analytical specificity: no cross-reactivity with other steroid compounds or supplements like biotin/

–   Analytical Measurement Range: 1.0 ng/dL to 2,000+ ng/dL

For women, children, and hypogonadal males, in whom it is expected very low testosterone concentrations, the advantages of LC/MS/MS technology are very important. LC/MS/MS can separate out interfering and structurely similar compounds. The cheaper and more common direct immunoassays can suffer significant cross-reactivity from testosterone-like compounds, causing erroneous results. Therefore, they are not recommended for low level testosterone quantitation.

In 2007, the Endocrine Society recommended use of extraction and chromatography followed by either immunoassay or mass spectrometry for measuring low testosterone concentrations. Direct immunoassays, however, are still suitable for measuring testosterone levels in adult males, in whom the levels are expected to be higher. In addition, immunoassays have typically a faster turnaround time.
